The Elders Home has the capability of servicing current and new residents with Hospice cares. Hospice cares are offered for the individual with a chronic condition in the current stages of, end of life. Diagnosis that Hospice services include, but are not limited to:

• Cancer
• Cardiac Disease
• COPD/Lung Disease

The Hospice of choice and the Elders Home collaborate to make an individualized plan of care, to support end of life needs including pain control, emotional and spiritual support, along with family support and bereavement services. Grief support services are also provided by the Hospice organization to assist the family with the transition after the passing of their loved one.
